Selection: You have been asked to participate in a graduate research study conducted by a student researcher attending Lincoln University's Master of Human Services program. This research involves the study of access to health care as part of a broader application of social research. You have been selected by a simple random sampling method. The overall study involve surveys, interviews, and observations. You are being asked to complete a short survey which may take approximately 5-10 minutes, scheduled at your convenience by your own on-line response time. Privacy: You will be afforded complete confidentiality in completing the survey. The informed consent forms and other materials will be kept (separated from unrelated documents) in locked file cabinets and on a secure computer at the researcher's office. Contact may be had at the address provided above. All research materials will be destroyed three years after the completion of the study. The results of this research will be published, without names, in the findings and possibly in subsequent peer-reviewed journals or books. Risks & Benefits: You may develop greater personal awareness of the value of your insights and life experiences as a result of your participation in this research. There are no risks to you; there is a small chance that you may experience some emotional discomfort during or after the survey. There is no penalty for not participating in this study. If you choose not to participate, you may withdraw from this study at any time, without negative consequences.
